Case Management
Case Management is a client centered service in which needs are identified and a plan of services is developed to address these needs in the preferred setting of the home. Case Management is provided by professionals with a background in aging issues. The Agency Case Manager’s are caring, patient and compassionate individuals with extensive knowledge in problem solving and community resources.
Medical Supply Assistance
For those who qualify, assistance may be provided in securing and paying for medical supplies. Supplies may include such items as Depends and/or wipes.
